Goodpasture's syndrome
Goodpasture's syndrome (Anti-GBM mediated glomerulonephritis)
The Goodpasture's syndrome is a rare combination of recurrent pulmonary hemorrhage and rapidly progressive inflammation of the filtration units of the kidneys (glomerulonephritis).
This relationship is the result that a relationship exists between the basement membrane antigen in the kidney and in the pulmonary alveoli.
Frequency: rare
Symptoms
- recurrent pulmonary hemorrhage, which may take under life threatening circumstances.
- Proteinuria (increased excretion of protein by the kidney) and hematuria (blood in the urine)
- rapidly progressive renal failure
Diagnosis
- Detection of Antibasalmembran antibodies in the kidney and lung biopsy
- hemorrhagic (bloody), radiographic infiltrates of the lung
Complications
- massive loss of blood through the pulmonary hemorrhage
- Renal failure
Therapy
- daily plasmapheresis in the early stages of the disease (replacement of part of the blood in which the antibodies are included).
- Immunosuppressive therapy with glucocorticoids and cyclophosphamide
- Treatment of anemia and the resulting high blood pressure
Forecast
Mortality of untreated disease high in premature initiation of therapy, the prognosis can be improved significantly
